This appraisal form is for physicians supervising basic physician trainees in the Greater Western Consortium, who have been asked by a trainee to complete an appraisal of him/her. It will take about 15 minutes to complete on a reliable internet connection.
Rotation appraisals are vital to help us improve the facilities and training opportunities within each health service. Please help us by filling in the following questions carefully.
Your responses will be kept confidential and no personally identifiable information will be disclosed outside of the Greater Western Consortium, unless by prior mutual agreement. Aggregated data will be statistically analysed and presented to clinical units, health services, the Department of Human Services and other Consortium stakeholders.
